Provide your primary any additional telephone numbers that will assist our committee in contacting you regarding your interview which will precede voting on your petition in open lodge. Your petition for degrees must be signed by two (2) member of Scioto Lodge #6 F. & A.M. Do not fill in any of the blanks near the top of the petition, those are for office use only. Be sure to answer the question regarding whether or not you have ever petitioned a Lodge of Free and Accepted Masons before. When signing your petition, be sure to also PRINT your name in full legibly on the line below where your signature appears. All information remains confidential and the petition, once completed, becomes the property of Scioto Lodge #6 F. & A.M. Submit your check or money order in the amount of $115.00 when petitioning for degrees.
